Rent a ventilator

REQUIREMENTS:

1. Prescription issued by a licensed US based clinician

2. Copy of Government Issued ID

3. Proof of Address

A Rental Agreement will be sent via DocuSign to your provided email that must be signed before release of equipment.

Security Deposit is required upon rental and will be refunded upon the return of the unit.
